                                                  Service Policy
Overview:
The following is an overview of services provided by American Medical Maintenance, LLC also known as AmeriMed . The purpose is to set the level of expectations for both parties and to serve as a quick reference for rates and exceptional level of service we provide.  
Regular Service Call:   within 24hr. response
Regular service calls are to be scheduled with our service department in conjunction with your office contact person and will be responded to by the end of the next business day.  All regular service calls are scheduled and performed Monday – Saturday from 8:00am to 5:00pm and are billed at the following rates.  
• Installation of equipment purchased through American Medical Maintenance includes labor—Extra fittings or parts will be an additional charge

Emergency Calls:
Emergency calls will be responded to within 4 hrs of the service department receiving the call at  no additional charges. Calls received after 1:00pm, may be scheduled as first call next morning. Emergency calls are defined as follows and will be assessed by our service department for priority:
• No air to treatment rooms    • No suction of treatment rooms
• No functioning method of sterilization
• No functioning method to develop x-ray films
• No functioning x-ray units in all treatment rooms

Warranty:
• 30 day parts & labor on repairs performed by AmeriMed
• We honor manufacturer warranties on equipment and parts purchased through AmeriMed
• AmeriMed will honor manufacture warranties as a service provider for that manufacture.
Shipping:
Do to the high cost of shipping all parts will be shipped via ground service and normal freight charges will be added to the service invoice.  In emergency situations, parts will be shipped next day air at customer's expense. 
Rentals:
We provide rental equipment at competitive rates.  These are intended as a short term use only until your equipment is repaired or replaced.  Should your equipment be repaired by AmeriMed or you decide to purchase replacement equipment though AmeriMed, your rental fees will be waved.

Rentals may not be an exact duplicate. Rentals will be billed weekly per device plus any applicable freight and/or delivery/pickup charges
. 

Loaner:
Loaners are provided till time of estimate of repair or till repairs are completed. If an approval for repair is not provided then rental rates become active. There is an installation charge of $65.00 for pick up and delivery and install of loaner equipment.
Holidays:
We will be closed on nationally recognized holidays
.
Service Agreement Length:
Service agreement will expire one year from date of signing.  Either party can cancel this agreement at any time.  Each year the contract is to be reviewed and updated prior to renewal.  All terms, rates, and privileges allowed under this agreement will be void if the account is not in current standing.
Sterilizer Validation Service:
American Medical Maintenance provides Validation Services according to AAMI st79 standards. All bench sterilizer repairs and Preventative Maintenance will include a validation report.
Billing:
AmeriMed asks that services incurred are paid upon receipt. Credit Card payments accepted.  AmeriMed billing in the following manner; in the first  half hour a standard $70.00 then the next 15 min to $1050.00 then to complete the hour with $140.00, after that hour in per min rate at $2.30....
AmerMed's billing reflects travel time and gas consumption.
Late Fee:
AmeriMed services require payment due on day of service. AmeriMed will extend payment due for 10 days as a courtesy to it's customer's. If payment is not received with in the grace period then a $30.00 late fee can and will be applied to the current Invoice.
